feel free to review them at your computer, or download to your printer for completion.  (click here)

Before completing the forms, simply refer to the PFG Account Matrix (page 3 of the Customer Agreement) to determine which forms you will be required to complete. Make sure to keep a copy for your records.

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you (we may also ask to see your driver's license or other
identifying documents).

Upon completion, fax THE COMPLETE SET of forms to:

Upon receipt, a representative from Platinum Capital Asset Management will contact you to schedule FEDEX (Federal Express) to pick-up the original account forms and paperwork for next day delivery to:
